     – A power of attorney is required only when acting by an agent. Eligible agents are as follows:
      • A statutory agent of a minor or adult ward;
      • An agent authorized by the individual for the purpose of making the request for Disclosure, etc.;
      • Where there is doubt as to the individual’s decision‑making capacity, a relative or a person equivalent thereto who actually cares for the individual.

d) Fees
A fee of 1,000 JPY (excluding tax), payable by postal fixed‑amount money order, is charged per request for Disclosure, etc. Please
enclose the fee together with the request documents when mailing them.
If it is determined that a ground for refusing disclosure prescribed by the APPI applies, fees already paid will not be refunded.

7. Security Control Measures
a) Organizational Security Control Measures
– We have established internal regulations concerning the protection of personal information and have developed a personal information protection framework, including appointing persons responsible for handling personal data, to ensure proper handling of personal data.
– We conduct periodic self‑inspections of the status of handling personal data and conduct internal audits by other departments or external parties.

b) Human Security Control Measures
– We execute confidentiality undertakings with all employees and provide periodic education on personal information protection.
– Matters concerning confidentiality of personal data are stipulated in our work rules.

c) Physical Security Control Measures
– In areas where personal data are handled, we manage entry/exit of employees and restrict equipment brought in, and implement measures to prevent viewing, etc., of personal data by persons without authority.
– We take measures to prevent theft or loss of equipment, electronic media, documents, etc., that handle personal data; and when carrying such equipment or media, including within the office, we implement measures to prevent personal data from being readily identified.

d) Technical Security Control Measures
– We implement access controls to limit, for each employee, the scope of personal data that may be handled.
– We have introduced mechanisms to protect information systems handling personal data from unauthorized access and malicious software from external sources.

e) Understanding of the External Environment
– Including, in relation to entrusted processing, we do not store personal data outside Japan.
